2. CHEMISTRY
Thiamine hydrochloride is 3-(4-amino-2-methylpyrimidin-5-yl)methyl)-5-(2-hydroxyethyl)-4-methylthiazolium chloride hydrochloride. Pyridoxine hydrochloride is 5-hydroxy-6-methylpyridine-3,4-dimethanol hydrochloride.
3. INDICATIONS
Thiamine is an essential coenzyme for carbohydrate metabolism. Thiamine deficiency occurs with inadequate dietary intake; severe deficiency results in a syndrome known as beriberi. Chronic’dry’beriberi is characterised by peripheral neuritis, muscle wasting, muscle weakness and paralysis. Acute’wet’ beriberi is characterised by cardiac failure and oedema. Demyelination of the central nervous system may occur in severe cases of thiamine deficiency.
Riboflavin is essential for energy utilisation from food. Active flavin mononucleotide and flavin adenine nucleotide function as coenzymes in oxidative/reductive metabolic reactions. Riboflavin deficiency develops from inadequate dietary intake, leading to a well-defined syndrome called ariboflavinosis, characterised by cheilosis, angular stomatitis, glossitis, keratitis and seborrhoeic dermatitis. Ocular symptoms may also present, including eye itching and burning, photophobia and corneal vascularisation.
Pyridoxine is mainly involved in amino acid metabolism, and also participates in carbohydrate and fat metabolism. It is also required for haemoglobin synthesis. Pyridoxine deficiency in adults leads to peripheral neuritis; deficiency in children can also affect the CNS. It is used for the treatment of sideroblastic anaemias and certain metabolic disorders.
Nicotinamide and nicotinic acid are naturally occurring vitamin B substances in the human body. They are converted into nicotinamide adenine dinucleotide (NAD) and nicotinamide adenine dinucleotide phosphate (NADP). These coenzymes participate in electron transfer reactions in the respiratory chain. Nicotinamide and nicotinic acid are used for the treatment and prevention of nicotinic acid deficiency. Deficiency arises from insufficient dietary intake and causes pellagra, characterised by skin lesions, especially on sunlight-exposed areas, with hyperpigmentation and hyperkeratinisation. Other symptoms include diarrhoea, abdominal pain, glossitis, stomatitis, loss of appetite, headache, lethargy, as well as mental and neurological disturbances.
D-Panthenol is the alcoholic analogue of D-pantothenic acid and is categorised as a vitamin B substance. It has been used in a variety of conditions, including the management of gastrointestinal atony.
4. CONTRAINDICATIONS
Contraindicated in haemophiliacs and patients with ileus caused by mechanical obstruction.
5. ADVERSE EFFECTS
Hypersensitivity or allergic reactions and other adverse effects rarely occur with long-term administration of large doses.
6. ADMINISTRATION
For adults: 2 ml or more daily via intravenous injection, or as directed by the physician.
7. STORAGE
Store at controlled room temperature (25℃/77℉) and protect from light. Keep all medicines out of the reach of children.
